Liturgical Ministry Sign-Up

When you sign up as a Liturgical Minister, you can submit your sign-up either electronically (via e-mail) or on paper (via U.S. Mail, or by hand delivery to the parish office).

In order to do electronic sign-up, you must have Microsoft Word or another application that supports files in the Microsoft Word format. Either the Windows or Mac version of Word will work.

  1. Save a copy of the sign-up form Word file to your local disk.

  2. Open the Word file and fill in your answers.

  3. Send your updated Word file as an e-mail attachment.

  1. View and print the sign-up form.
    Adobe Acrobat Reader, or other PDF file viewer, is required to view the form; or you may download and print the Word file instead of the PDF file.

  2. Fill out the printed form with your answers.

  3. Mail or deliver the completed form to:
